CAGE Questionnaire This simple 4-question self-test may help you become aware of your use or abuse of alcohol. This test specifically focuses on alcohol use, and not on the use of other drugs. The CAGE questionnaire was developed by Dr. John Ewing, founding director of the Bowles Center for Alcohol Studies, University of North Carolina at Chapel Hill. CAGE is an internationally used assessment instrument for identifying problems with alcohol. 'CAGE' is an acronym formed from the italicised letters in the questionnaire (cut-annoyed-guilty-eye). After heavy drinking/using, have you ever had delirium tremens (D.T.'s), severe shaking, heard voices, or seen things that weren't there? Scoring: Yes=2 19. Have you ever gone to anyone for help about your drinking/using? Scoring: Yes=5 20. Have you ever been in a hospital because of drinking/using? Scoring: Yes=5 21. Have you ever been in a psychiatric hospital or ward where drinking/using were part of the problem that resulted in hospitalization? Scoring: Yes=2 22. Have you ever been seen at a psychiatric hospital or mental health clinic or gone to a doctor, social worker, or clergyman for help with an emotional problem in which drinking/drugs were part of the problem?
